Vetcetera Pet Healthcare Centre offers our patient form(s) online so you can complete it in the convenience of your own home or office. Click on each form to open or download.
New Clients/Pets: If you are a new client, please fill out a Client Registration form, and a Pet Health History form for your pet. However, if you are already a client and are bringing in a new pet, simply fill out the pet health history form.
Pet Surgery: If your pet is scheduled to receive surgery from us, please open and read the Surgery Protocol Information sheet, then download the Surgery Admission form. Fill it out the form either on your computer or by hand, then print and sign the form to bring with you to your next appointment.
